Statement of Purpose Summary
The service provides support to individuals within their own homes, both on a practical and personal care level, which enables them to maintain their own independence and lifestyle. We aim for our services to be delivered in a professional and friendly manner.
We offer services to people who experience relative disadvantage as a result of frailty, illness or disability: dependence or lack of support. Some of the services on offer provide practical support to living in their own homes; to minimize risks to their health and safety and to sustain their independence.
Philosophy of Care:
We aim to provide a tailored service to meet your assessed requirements, to promote dignity, privacy, confidentiality rights and choice, helping you to maintain maximum quality of life and independence in your own home environment. We provide a service that supports you in a professional and confidential way.
We aim to provide staff that are skilled to perform the desired tasks and to ensure staff are trained and updated regarding care skills. We are committed to training and development of staff.
Your needs are assessed by completing a personalised assessment in your own home setting, which is tailored made to suit your individual needs.
Night support including ‘put to bed’ and ‘get up’ service.
This is very often a very difficult time of day for carers, friends and family members, especially if they have responsibilities of their own. It may be that additional systems are required to ensure independent living and a call in a morning or evening may be all that is required to ensure this occurs. Support is provided based on the needs of the individual and can include personal care tasks and also includes emptying of commodes and making the bed up if required.
Following an assessment of individual needs and bathing facilities available within the home, assistance can be offered with bathing/showering. It may involve the use of bathing aids and staff can support the individual within their home to maintain this action, this would be discussed during assessment and following a risk assessment.
Respite Care and Carer Relief
Being a Carer can be time-consuming and tiring, which is why we offer a Carer Relief Service. This could mean a staff member sitting in the clients home whilst the Carer/Family Member does errands or socializes with friends. It may be appropriate, with weather permitting and an individual risk assessment completed, that the individual goes out for the day with the staff member. There is also the facility to support clients and Carers by offering a respite service whereby staff would sit with clients whilst the Carer/Family Member goes out for the evening. This can be extended to a night sitting service to ensure Carers/Family Member get a full nights rest. We can provide peace of mind day or night. Holiday cover may be something that may help the carer to re-charge their batteries and ensure they have a break with the assurance that the care being delivered to their loved one is to the highest possible standard.
Assistance getting to appointments
Embec Care has a specially adapted People Carrier that has a ramp for wheelchair access so we can easily transport a wheelchair user and four other passengers to all kinds of appointments such as Doctors, Dentists and Hospital etc or perhaps visits to friends, relatives or shopping trips etc.
Light Housework Tasks
This may include tasks such as vacuuming, dusting, ironing, washing and changing bed linen.
We recognize that personal care is a private and personal thing to each individual. All staff ensure privacy and dignity at all times allowing the individual to feel comfortable and confident with the support being provided. Individual assessments are completed in individual's home at a time to suit everyone involved. It may be possible to support personal cleansing tasks, getting dressed or undressed and we offer some support with changing catheter bags or assisting with the changing of T E D's stockings or other compression hosiery. Personal care tasks also include; washing, shaving, oral care and toileting.
Advice in completion of forms
The completion of forms can be most daunting and off-putting. We have staff that will help with the completion of benefit claim forms such as Attendance Allowance, Disability Living Allowance and Pension Credit to name just some. We will also guide individuals down the path of direct payments which will help in the purchasing of care.
This can be supported in two ways, either by the staff member collecting a shopping list and money and shopping at the individual's local store, or taking the individual shopping to the local store so they can not only choose their own grocery but also benefit from some local interaction. This can also support nutritional requirements, special diets and meal preparation.
We aim to provide support during times of crisis and are and we will try to facilitate assessment and staff placement quickly.
We now offer 24 hour cover on a shshort-term basis providing support and peace of mind to families knowing your loved ones are being looked after in their own homes while you are away, an individual assessment will be done to create a care package ideally suited for each individual, then we will introduce an experienced member of staff before care commences, ensuring the individual and carer are happy with the proposed care package. Holiday cover could be for the weekend or for a max of two weeks. The cost of holiday cover varies depending upon the needs of the individual and whether there is a hoist in situ or other equipment is in place.
We can support on discharge from the hospital, this maybe to offer support following a spell in hospital whilst you get stronger and to provide family reassurances.
We can help prevent a hospital admission by providing emergency cover up to 24hrs.
This service is available to clients and families who require friendship and companionship during an afternoon.

All our services are delivered to the highest standards, to ensure that you receive the best care possible every client will be individually assessed and a care plan agreed.
In addition, all staff have an enhanced police check [DBS].

Part of the service we offer is to evaluate your needs and if you qualify for any Government funding towards your care we’ll help you fill in those complicated forms that may cover the cost of your care.
